To request a copy of your medical records or to give another person access to your records, patients can submit an Authorization for Release of Medical/Billing Information online, by fax, by mail, or in-person. However, many of your records are already available on the Ashe Patient Portal and do not require prior authorization to view.
Records Available on the Patient Portal
Patients can access the records below (April 2016-present) without a written authorization by logging into the Patient Portal:
- Visit History
- Allergies
- Medications
- Radiology reports
- Lab test results
- Procedures
- Immunizations
- Referrals
- Letters
- Secure Messages
- Itemized Statements
For All Other Records
Patients needing records prior to April 2016, or needing more detailed information, can submit a request by using one of the options below:

- Allow 7-10 business days for your request to be completed. Unsigned and/or incomplete requests cannot be processed.
- There is no fee for current or past students.
- For third parties (Outside Provider, Law Firm, or Facility), there is a $15 processing fee, please address the check or money order to “UC Regents.”
