Medical Records

To request a copy of your medical records or to give another person access to your records, patients can submit an Authorization for Release of Medical/Billing Information online, by fax, by mail, or in-person. However, many of your records are already available on the The Ashe Center Patient Portal and do not require prior authorization to view.

Records Available on your Patient Portal

Patients can access the records below (April 2016-present) without a written authorization by logging into the Patient Portal:

Please Note

  • Allow 15 business days for your request to be completed. Unsigned and/or incomplete requests cannot be processed.
  • There is no fee for current or past students.
  • For third parties (Outside Provider, Law Firm, or Facility), there is a $15 processing fee, please address the check or money order to “UC Regents.”
  • To request a copy of your X-ray imaging study, follow these instructions from UCLA Health.
