What to Expect

Our Health Clearance Clinic provides a statement of health or medical clearance forms signed by a physician or nurse practitioner for graduate schools, study abroad, employment, or volunteer work.

The appointment does not include a physical exam.

Health Clearances are processed at the 3rd floor Clinic Station.

DOT, CMV & Immigration Exams

Patients who need a Department of Transportation (DOT) medical exam and commercial motor vehicle (CMV) certificationmust be seen by a licensed “medical examiner” listed on the Federal Motor Carrier Safety Administration (FMCSA) National Registry. Find a medical examiner who is certified by the FMCSA to perform DOT physical exams.
Patients who need an immigration medical exam, see the following table to find an approved doctor near you.
