Students who are unable to attend work or class, or unable to complete an assignment, examination or presentation due to acute medical condition, can request a medical excuse note from The Ashe Center Patient Portal.
Requesting a Medical Excuse Note on the Patient Portal:
From The Ashe Center Patient Portal main page, please follow the below steps:
Messages > New Message > Request a Medical Excuse > Ashe Medical Excuse and enter the necessary information.
A Note on Medical Excuses
Self-generated medical excuse notes cannot exceed one week. If you were/are unable to attend work or class, or unable to complete an assignment, examination or presentation for one week+, please schedule an appointment with your Ashe Center primary care provider to have your symptoms evaluated.
